The public health message around high blood pressure has been to eat less salt. But a new study suggests that advice may be missing half the story.

In a perspective paper published in The American Journal of Clinical Nutrition, scientists say the real issue may be the sodium-potassium balance in our diets. While too much sodium can push blood pressure higher, not getting enough potassium could be making the problem worse.

The global salt problem is still getting worse

The World Health Organization recommends adults consume less than 2,000 milligrams of sodium per day and get more than 3,500 milligrams of potassium daily. The reality is far from that goal. Across the world, people consume roughly twice the recommended amount of sodium, while potassium intake often falls far below recommended levels.

And no, the biggest culprit usually isn’t the salt shaker sitting on your kitchen table. For many people, most sodium comes from foods bought outside the home, especially packaged meals, processed foods, and restaurant dishes. For clarity, your fries, instant noodles, frozen meals, and takeout orders may be doing more heavy lifting than that tiny container of table salt.

Potassium needs more attention

Researchers led by University of Vermont professor Naomi Fukagawa argue that focusing only on reducing sodium has not been enough. The reason is that people like salty food a lot. Food companies also face challenges reformulating products because changing salt levels can affect taste, texture, and preservation.

Meanwhile, evidence supporting potassium’s role in blood pressure control has continued to grow. The researchers argue that public health strategies should stop treating sodium as the only problem and start looking at the relationship between the two nutrients.

“Food components interact and physiology is integrative across systems,” Fukagawa said, emphasizing that nutrition advice needs a different approach.

Our kidneys are basically running a sodium-potassium negotiation

The science behind this comes down to how the human kidney manages these minerals. Researchers point to a system called the renal potassium switch, which helps the body balance sodium and potassium while producing urine.

Historically, this system evolved when humans struggled to get enough potassium from food. The body learned to hold onto potassium and manage sodium carefully. The problem is that modern diets flipped the script. Today, many people consume plenty of sodium but not enough potassium-rich foods. That mismatch can push the body toward higher blood pressure.

The fix might be more on your plate than in your salt shaker

One major piece of evidence comes from the China Salt Substitute and Stroke Study. Researchers found that replacing some regular salt (sodium chloride) with potassium-based salt substitutes lowered blood pressure and reduced cardiovascular events.

According to scientists, much of the benefit came from adding potassium and not just cutting sodium.

They also suggest several ways to tackle the problem, one of which is increasing access to potassium-rich foods like fruits and vegetables. Food companies are also encouraged to rethink sodium and potassium levels together while individuals are advised to expand the use of potassium-based salt substitutes where appropriate.

There is a catch, however. Potassium substitutes are not suitable for everyone. People with advanced kidney disease or those taking certain medications may need to avoid increasing potassium without medical advice. So before swapping your regular salt for a potassium alternative, check with a healthcare professional.
